Special Registration Options for Critical Access Hospitals:


There is a $200 discount for those who work at a CAH, but you must follow these instructions to be eligible:

  1. Download and print this special registration form.

  2. The #200 indicates that the discount will be applied. Complete the special printed registration form with your information and select your desired sessions, the appropriate rate and payment information on form.

  3. Fax this form to fax number 708-531-0665 and save your copy.

  4. Confirmations will be sent to you shortly after registration.
