Special Registration Options for Critical Access Hospitals:
There is a $200 discount for those who work at a CAH, but you must follow these
instructions to be eligible:
- Download and print this special
registration form.
- The #200 indicates that the discount will be applied. Complete the special
printed registration form with your information and select your desired sessions,
the appropriate rate and payment information on form.
- Fax this form to fax number 708-531-0665 and save your copy.
- Confirmations will be sent to you shortly after registration.
